  3. Diagnosis

    • Provide a comprehensive and accurate diagnosis based on the assessment findings and any additional tests or imaging results.

  4. Management Plan

    • Develop a personalized management plan tailored to the specific needs and goals of the patient.

    • Discuss the timeline for recovery, including short-term and long-term goals.

    • Outline the steps and interventions required for effective treatment and recovery.

  5. Injury Management Discussion

    • Review and discuss any other inputs or factors that may impact injury management, such as lifestyle, work, and activity levels.

    • Provide education on injury prevention and self-management strategies.

  6. Rehabilitation Plan

    • Create a customized rehabilitation plan, including specific exercises and therapeutic interventions designed to promote healing and restore function.

  7. Summary and Follow-up

    • Send a detailed email summary of the assessment findings, diagnosis, management plan, and rehabilitation plan.

    • Include exercise videos or other resources as needed to support the rehabilitation process.

    • Schedule follow-up appointments to monitor progress and adjust the treatment plan as necessary.

Acute Initial Assessment 

For injuries brought on through an accident that are less than two weeks old.

    • Provide referrals to appropriate specialists, consultants, or imaging services if further investigation or specialized care is needed.

    • Offer a clear and accurate diagnosis based on the assessment findings and any additional tests or imaging results.

  4. Management Plan

    • Discuss a concise management plan tailored to the patient's immediate needs.

    • Outline a timeline for recovery, including key milestones and expected progress.

    • Explain the initial steps and interventions required for effective treatment and recovery.

  5. Rehabilitation Plan

    • Develop a preliminary plan to begin rehabilitation, including initial exercises and therapeutic interventions aimed at reducing pain and promoting healing.

    • Provide guidance on self-care and injury prevention strategies to support the recovery process.

  6. Summary and Follow-up

    • Summarize the assessment findings, diagnosis, and management plan in an easy-to-understand format.
